I've written two versions of a story (The Assistant) where the secretary shows an older girl (one part of a mentoring program) how the corporal punishment system works, form wise. Of course they needed a name to use on the forms and mix ups can happen...
In that story it would have been very easy for that secretary to either discard or a forge a CP slip, saving or dooming a student as she did so.
